Background data: Posterior cervical laminectomy and lateral mass screw-rod fusion techniques were classically recommended in flexible sagittal alignment relying on indirect decompression via posterior cord shift. Purpose: This study aims to investigate the efficacy of with fixation for treating multisegmental spondylotic myelopathy alignment. Study Design: Prospective clinical cohort study. Patients Methods: In total, 38 patients clinically symptomatic M-CSM instability and/or kyphosis admitted our Zagazig university hospitals (long-segment instrumented ≥3 segments) completed 24-month follow-up period between April 2014 June 2018 last visit took place October 2020. categorized into lordotic, straight, kyphotic groups according shape spine curve a neutral X-ray view.
